A5: Waiver Application Forms

There are two Waiver Application Forms.

The first is for use where the applicant is an individual - W1

The second is for use where the applicant is a practice unit - W2

Fees - there are no fees applicable to either of these forms.

Please download the appropriate form to the right, complete it, print it out, sign it and return it to us by post or email addressed to the Secretary of the relevant Sub-Committee, as detailed below:
