D4: Practice Unit Information

Initial notification, annual confirmation and changes

New practice units

4.1.1 The managers of a practice unit which commences practice after the date these rules come into operation shall ensure that the Council is informed of such commencement of practice and certain information in relation to the practice unit and those within it within 7 days of the date of such commencement by the completion and submission (on behalf of the practice unit) of an initial notification form in such form as may be prescribed from time to time by the Council.

Existing practice units 

4.1.2 The Council may issue forms seeking confirmation that the information similar in nature to that to be supplied in respect of new practice units pursuant to rule 4.1.1 held by the Council in respect of practice units which commenced practice prior to the coming into operation of these rules remains complete and correct.  Such forms shall be as prescribed from time to time by the Council. The managers of each practice unit shall ensure that any form so issued to the practice unit is completed and submitted to the Council (on behalf of the practice unit) within 28 days of its receipt.

All practice units 

4.1.3 No more than once in each practice year the Council may issue a confirmation form to each practice unit.  Such form shall seek confirmation that the information held by the Council provided pursuant to rules 4.1.1 or 4.1.2 remains complete and correct and shall take such form as may be prescribed from time to time by the Council. The managers of each practice unit shall ensure that any confirmation form so issued to the practice unit is completed and submitted to the Council (on behalf of the practice unit) within 28 days of its receipt. 

4.1.4 The managers of every practice unit shall ensure that the Council is advised of any change to any information submitted to the Council pursuant to any of rule 4.1 within 28 days of the effective date of such change by the completion and submission (on behalf of the practice unit) of the appropriate change notification form (which shall be in such form as may be prescribed from time to time by the Council).

Notification of cessation of trading

4.2.1 The managers of a practice unit which they intend to cease trading shall ensure that the Council is informed of that intention not less than 28 days prior to the planned date of cessation by the completion and submission (on behalf of the practice unit) of a cessation notification form in such form as may be prescribed from time to time by the Council.  

4.2.2 Every managers of a practice unit to which rule 4.2.1 applies shall provide such further information to the Council as the Council may reasonably request within such periods as the Council may reasonably stipulate, all in order that the Council may ascertain that the managers and the practice unit are complying with these rules and have made appropriate arrangements to ensure compliance by the practice unit, former practice unit and any of its managers or former managers with such of these rules as will continue to apply to such parties following the cessation of trading intimated pursuant to rule 4.2.1.
